• Full Time position – SCHADS 5.1 - $47.50 per hour 
• Baulkham Hills office base with travel to carers homes
• Foster Care/Out of Home Care experience preferred

The role

The role of the case manager is to coordinate all aspects of case management support to children, young people and their families and/or carers to ensure that all of their holistic needs are met and their rights are upheld. The case manager will be responsible for the day to day care responsibility of children in the foster care system with Lifestyle Solutions, which includes:

• Completing and consistently reviewing annual out of home care case plans that focus on ensuring the holistic needs of children and young people are met

• Upholding the rights of children, young people and their families and carers.

• Focusing on ensuring the permanency and consistency of care of Children in the foster care system

• Completing and reviewing cultural plans where appropriate to ensure that children and young peoples cultural needs are enhances and have knowledge of and are connected to their family heritage and culture

• Engaging in regular home visiting for children, young people and families/carers to monitor children's progress and hear their voice

• Completing legal and court work that that meets legislative requirements through the relevant state legislation.

• Assess the safety and risk of children and young people in out of home care.

• Modeling a culture of social justice and advocacy on behalf of the agency, programs and those we support championing restoration, kinship placement and family lead decision making

• Engaging in supervision with the line manager, and through group supervision.

• Build and maintain external relationships within the area, including government, funder, community and corporate stakeholders

• Take an active role in planning and delivering community events in the effort of Carer recruitment and community education around out of home care 

About you

As an experienced Case Manager, you will:

• Manage a caseload of children and young people/adults and their families and carers
• Complete comprehensive assessments to ensure the development, implementation and review of individual case plans
• Drive intervention strategies e.g., risk assessments, referrals and program applications
• Apply an understanding of child protection related court procedures and practices, including preparing court testimonies and other legal documentation
• Facilitate access to services and provide feedback on their effectiveness
• Prepare reports and correspondence relating to the implementation and progress of case plans
